Having a parent with diabetes, with diabetes in two or more generations. Not necessarily needing insulin. MODY is very rare compared with type 1 and type 2 diabetes – experts The latter is the hallmark of MODY and distinguishes it from type 2 diabetes. MODY can be diagnosed at a young age and frequently in childhood or early adolescence (9–14 years of age or earlier), particularly if sought by routine OGTT.

Maturity-onset diabetes of the young (MODY) is a heterogenous group of monogenic causes of β-cell dysfunction that leads to diabetes presenting in young adults. Those with MODY have diabetes characterised by: young age at onset (10–45 years) autosomal dominant family history MODY with the aim of raising awareness of this condition amongst health-care scientists.

The clinical diagnosis of MODY is usually based on the following criteria: autosomal inheritance of diabetes, insulin independence, and age at onset younger than 25 years .

1 As knowledge of the condition has expanded, the initial criteria outlined (autosomal dominant inheritance, age of diagnosis <25 years and no insulin requirement) have been found to recognise some, but not all cases of MODY. 2 Maturity-onset diabetes of the young (MODY) is a suspected diagnosis in young non-obese patients who lack an autoimmune cause for diabetes and who have a family history of diabetes in successive generations. The majority of MODY cases are due to mutations in one of four genes. Nonetheless, new mutations can occur in individuals without family history of MODY. In favor of this diagnosis there is the strong fact that abnormal pancreatic development was present, which would be a strong reason for diabetes mellitus, particularly MODY types 5-8 . In our patient multiple gene screening for MODY was performed. Depends on MODY type Insulin Resistance Mostly no; ~10% ,yes Some Yes Depends on MODY type Presence of Autoantibodies Yes (ICA, IA2, GAD65, IAA) Yes (mostly GAD65), Some not No No T cell Reponses to islet proteins Yes Yes No No Insulin/ C-peptides Level at diagnosis Undectable or extermely low Low Normal to High Normal Maturity-onset diabetes of the young (MODY) includes several forms of diabetes with monogenetic defects of beta-cell function (impaired insulin secretion), usually manifesting as mild hyperglycaemia at a young age and usually inherited in an autosomal-dominant manner .

For adults who do not need immediate insulin treatment, a default diagnosis of type 2 diabetes is the most common outcome, but a small proportion of patients will actually have maturity-onset diabetes of the young (MODY; a type of monogenic diabetes). MODY is inherited in an autosomal dominant pattern and estimated to account for 1–2% of all diabetes.1 MODY is most commonly caused by mutations in genes encoding the transcription factors HNF1A , HNF4A , and HNF1B , and the glycolytic enzyme MODY is a rare cause of diabetes (1% of all cases) and is frequently misdiagnosed as Type 1 diabetes (T1DM) or Type 2 diabetes (T2DM). A precise molecular diagnosis is essential because it leads to optimal treatment of the patients and allows early diagnosis for their asymptomatic family members. The most common form of monogenic diabetes is maturity-onset diabetes of the young (MODY), which represents about 2% of all diabetes cases.
